Data controller

Renaissance Dental, Gustav III:s Boulevard 46, 169 73 Solna, is the data controller for the personal data we process when you use our website or receive care from us. Legal name and organisation number: Amidenta AB, org. no. 556841-9450.

What data we collect

• Contact form: your name, phone number, email address and the message you write.
• Booking: the details needed to book your visit.
• Visits and patient records: information about your health and your treatment. Records are kept in accordance with the Patient Data Act (patientdatalagen).

Why we process the data and on what legal basis

• To fulfil our care agreement with you: to book, carry out and follow up your care.
• To meet legal requirements, for example the duty to keep patient records under the Patient Data Act.
• With your consent when you submit the contact form, so that we can reply to your message. You give consent by ticking the box, and you can withdraw it at any time.

How long we keep the data

• The website does not store the message from the contact form itself. It is sent to the clinic as an email. We keep a log of emails sent (time, recipient and whether the email went out), but not their content.
• Patient record data is kept under the Patient Data Act, for at least 10 years.

Who we share data with

We may share data with:

• IT and email providers that help us run the website and receive messages. They may only process the data on our instructions.
• The Swedish Social Insurance Agency (Försäkringskassan), when you use the state dental care subsidy.
• National health data registers, when the law requires it.
• Our payment partner Human Finans, if you choose to pay in instalments. You apply to Human Finans yourself. We share only what is needed, for example the amount of the treatment.

Your rights

You have the right to:

• know what data we hold about you and receive a copy of it (access)
• have incorrect data corrected
• have data erased, in certain cases
• request that processing is restricted, in certain cases
• object to processing that is based on a balance of interests
• receive the data you have given us in a readable format (data portability)
• withdraw your consent at any time. This does not affect processing carried out before you withdrew it.

We must keep some patient record data by law, even if you request erasure.

Do not enter sensitive information in the form

Do not write about your health, diagnoses or other sensitive information in the contact form. Please call us on 08-650 10 60 instead, and we will talk about it securely.

Complaints

If you think we are handling your data incorrectly, you can complain to the Swedish Authority for Privacy Protection (Integritetsskyddsmyndigheten, IMY) at imy.se.
